Here, you can report foodborne incidents such as illness, foreign object, injury, allergic reaction, mislabeling, and off taste, color, or appearance involving meat, poultry, and processed egg products regulated by the USDA FSIS. The information you are able to provide in this form will help to support FSIS analysts in their ability to identify and respond to consumer food safety issues.

The valid OMB control number for this information collection is 0583-0133. The time required to complete this information collection is estimated to average 15 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information.
